This is a used Canon SUP-NS3 Third Generation lens support for new generation large Canon, Fujinon, or Angenieux Box lenses, such as XJ95, XJ80, XJ75, and XJ100 box lenses. It has a regular 12-pin connector for Sony or Panasonic cameras.
This item is very similar to the Canon SUP-NS2 and Fujinon ELH-112A-17A lens supports. There are some scratches near the 36-pin connector, which are purely cosmetic.
